Abstract Objectives: to analyze a decade of spatio-temporal behavior of pertussis in Brazil and its epidemiological characteristics. Methods: ecological time series study of pertussis cases and deaths from the Notifable Diseases Information System in Brazil (2010-2019). The method of generalized linear analysis of Prais-Winsten and the Kernel analysis were used. Results: 32,849 cases were reported, of which 466 (1.42%) evolved to death, with a prevalence of 1.63/100,000 inhabitants and a mortality rate of 0.023/100,000 inhabitants. In the temporal analysis, the cyclical behavior of pertussis was evidenced, with trend variations in the period in 2014. Most cases occurred in children under 1 year of age (60.16%, p<0.01), sex female (55.28%, p=0.066) and white (48.42%, p=0.14). The largest share of deaths was in children aged <1 year (98.07, p<0.01), females (56.01%, p=0.066) and whites (43.78%, p=0.14). In the Kernel of prevalence, the South, Southeast and Northeast regions stood out with high density; while for mortality, the Southeast and Northeast stood out. Conclusions: the cyclical behavior of pertussis was observed, with a decreasing trend in recent years and the concentration of cases in children. This reinforces the importance of strengthening the population's immunization process.


Resumo Objetivos: analisar uma década do comportamento espaço-temporal da coqueluche no Brasil e as suas características epidemiológicas. Métodos: estudo ecológico de série temporal dos casos e óbitos por coqueluche do Sistema de Informação de Agravos de Notificação no Brasil (2010-2019). Utilizou-se o método de análise linear generalizada de Prais-Winsten e a análise de Kernel. Resultados: notificaram-se 32.849 casos, desses 466 (1,42%) evoluíram para óbito, com prevalência de 1,63/100.000 habitantes e coeficiente de mortalidade de 0,023/100.000 habitantes. Na análise temporal, evidenciou-se o comportamento cíclico da coqueluche com variações de tendência no período em 2014. A maioria dos casos ocorreu em menores de 1 ano (60,16%, p<0,01), sexo feminino (55,28%, p=0,066) e brancos (48,42%, p=0,14). A maior parcela dos óbitos foi em crianças <1 ano (98,07, p<0,01), sexo feminino (56,01%, p=0,066) e brancos (43,78%, p=0,14). No Kernel da prevalência, destacaram-se as regiões Sul, Sudeste e Nordeste com alta densidade; enquanto para mortalidade, sobressaíram-se Sudeste e Nordeste. Conclusão: observou-se o comportamento cíclico da coqueluche, com tendência de decréscimo nos últimos anos e a concentração de casos no público infantil. O que reforça a importância de fortalecer o processo de imunização da população.

Objetivo: Analisar o impacto da vacina adsorvida difteria, tétano e coqueluche (pertússis acelular), tipo adulto (dTpa), materna, na incidência de coqueluche em crianças menores de 1 ano no Brasil, de 2008 a 2018. Métodos: Estudo de tipo ecológico, descritivo de tendência temporal, utilizando-se de dados dos sistemas de vigilância do Departamento de Informática do Sistema Único de Saúde (Datasus). Foram calculadas taxas de incidência e razão das taxas de incidência (IRR), com respectivos intervalos de confiança de 95% (IC95%). Resultados: O número de casos de coqueluche foi de 20.650. Houve redução na incidência de coqueluche no período pós-vacinal, de 26,6% (IRR = 0,73; IC95% 0,66;0,82) nas crianças de 3 meses a 1 ano incompleto, e de 63,6% (IRR = 0,36; IC95% 0,15;0,58) nas crianças de zero a 2 meses. Conclusão: O aumento da cobertura da vacinação com dTpa materna correspondeu à redução na incidência de coqueluche, principalmente na faixa etária de zero a 2 meses.


Objetivo: Analizar la incidencia de tos ferina en menores de 1 año y su relación con la cobertura de vacunación materna por la vacuna adsorbida contra la difteria, el tétanos y la tos ferina (tos ferina acelular) tipo adulto (dTpa), de 2008 a 2018, en Brasil. Métodos: Estudio ecológico descriptivo de tendencia temporal utilizando datos de los sistemas de vigilancia del Departamento de Informática do Sistema Único de Saúde (Datasus). Se calcularon los tasas de incidencia y las razones de tasas de incidencia (RTI) con los respectivos intervalos de confianza del 95% (IC95%). Resultados: El número de casos de tos ferina en el período de estudio fue de 20,650. Hubo una reducción en la incidencia de tos ferina en el período post-vacunación. En los niños de 3 meses a 1 año incompleto, la reducción fue del 26,6% (RTI = 0,73; IC95% 0,66;0,82), mientras que en el otro grupo la diferencia relativa fue del 63,6% (RTI = 0,36; IC95% 0,15;0,58)]. Conclusión: La vacunación materna con dTpa coincide con una reducción en la incidencia de tos ferina, especialmente en el grupo de edad de cero a 2 meses.


Objetive: To analyze the impact of maternal vaccination coverage with diphtheria-tetanus-acellular pertussis (Tdap) adsorbed vaccine for adults on pertussis incidence in children under 1 year old in Brazil from 2008 to 2018. Methods: This was a descriptive ecological temporal trend study using data from surveillance systems managed by the Brazilian National Health System Information Technology Department (DATASUS). Incidence rates and incidence rate ratios (IRR) were calculated with respective 95% confidence intervals (95%CI). Results: There were 20,650 pertussis cases in the study period. In the post-vaccination period there was a 26.6% reduction (IRR = 0.73; 95%CI 0.66;0.82) in pertussis incidence among children aged from 3 months up to but not including 1 year old, and a 63.6% reduction (IRR = 0.36; 95%CI 0.15;0.58) among children from birth to 2 months old. Conclusion: The increase in maternal Tdap vaccination coverage coincided with a reduction in pertussis incidence, especially in the birth to two-month-old age group.

Resumen A pesar de que la tosferina (coqueluche) es una enfermedad prevenible por vacunas (EPV), la epidemiología latinoamericana muestra que hay persistencia de brotes en la región. Esta persistencia se debe, al menos en parte, a factores tales como la cobertura vacunal, la presencia de movimientos anti vacunas, la diversidad de los sistemas locales de vigilancia y la falta de una definición de caso unificada para la región. Dada la importancia de la tosferina en Latinoamérica y los cambios ocurridos en las recomendaciones para la vacunación, este manuscrito tiene como objetivo revisar los datos epidemiológicos y los cambios recientes en los calendarios de vacunación y su impacto sobre la enfermedad pediátrica por Bordetella pertussis en Latinoamérica. Los datos epidemiológicos más recientes muestran que entre regiones, países, y segmentos dentro de cada país hay heterogeneidad en la cobertura vacunal, con distintos rebrotes. Esfuerzos en la región han tratado de mejorar esta situación al introducir vacunas acelulares (aP), menos reactogénicas que las vacunas de células enteras (wP) en los calendarios vacunales. Además, algunos países han mejorado la definición de caso confirmado, al introducir la reacción de polimerasa en cadena (RPC) como criterio diagnóstico. En respuesta a las heterogeneidades de cada país y a la epidemiología actual de la región, un Comité de Expertos de la Sociedad Latinoamericana de Infectología Pediátrica (SLIPE) y la Asociación Latinoamericana de Pediatría (ALAPE) propone una definición unificada de caso y recomendaciones para mejorar la cobertura vacunal y reducir los brotes de tosferina en Latinoamérica.


Abstract Although whooping cough is a vaccine-preventable disease (VPD), its epidemiologic characteristics in Latin America shows persistence of outbreaks in the region. This persistence is due, at least in part, to the presence of antivaccine movements, the diversity of the surveillance systems, and the lack of a uniform case definition for the region. Given the importance of whooping cough in Latin America and the changes in vaccine recommendations, this manuscript aims to review epidemiologic data and recent changes in the vaccination calendars and their impact on the pediatric disease by Bordetella pertussis in Latin America. Recent epidemiological data reveal that between regions, countries, and administrative units within each country there is a marked heterogeneity of vaccine coverage, with different outbreak patterns. Efforts in the region have tried to improve this situation by introducing acellular pertussis vaccines (aP) in the vaccine calendars, which are less reactogenic than whole-cell pertussis vaccines (wP). Moreover, some countries have improved the case definition. Some countries have implemented a confirmed case definition by introducing polymerase chain reaction (PCR) as a diagnostic criterion. As a response to the heterogeneities observed within and between countries and the regional epidemiologic profiles, a Steering Committee from the Latin American Society for Pediatric Infectiology (SLIPE) and the Latin American Association of Pediatrics (ALAPE) propose a unified case definition and recommendations to improve vaccine coverage and reduce the outbreaks of whooping cough in Latin America.

Justificativa e Objetivos: Apesar dos esforços despendidos pelo Programa Nacional de Imunizações, a coqueluche é um agravo que ainda gera grandes preocupações para a saúde pública. Assim, o objetivo deste estudo foi descrever as evidências na literatura sobre coqueluche na infância que indicassem as práticas de assistência à saúde necessárias para sua prevenção, promoção e controle. Método: Revisão integrativa da literatura nas bases de dados LILACS, MedLine, PubMed, SciELO, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL) utilizando os descritores Whooping Cough, Epidemiology, Vaccination. A revisão foi realizada em maio de 2020 e abrangeu a literatura disponível na íntegra, publicada em português, inglês ou espanhol, sem delimitação de ano. Resultados: Dentre os 1.248 artigos selecionados apenas nove foram analisados (a maioria norte-americana), dos quais 55% eram estudos realizados para prevenção de coqueluche. Quanto à autoria dos estudos, em 67% das publicações havia participação de um epidemiologista. Conclusão: Esta investigação mostra as evidências científicas sobre a coqueluche na infância, necessárias para prevenir, controlar, promover a assistência e definir o perfil epidemiológico desse agravo nessa população. Os resultados revelaram nível de evidência cinco, sugerindo que as pesquisas desenvolvidas na área não retratam fortes evidências.(AU)


Background and Objectives: Despite the efforts of the National Immunization Program, pertussis remains a serious health problem. Thus, this study described the evidence in the literature on pertussis during childhood, which elucidates the health-care practices necessary for prevention, promotion, and control of the disease. Methods: This is an integrative literature review conducted in the LILACS, MedLine, PubMed, SciELO, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL) databases, using the following descriptors: "Whooping Cough", "Epidemiology", and "Vaccination". The search was restricted to humanities, in English, Portuguese, or Spanish, without year delimitation. The search was completed on May 2020, Results: in total, 1,248 articles were selected, of which only nine articles were analyzed, mostly from North America, and 55% of the analyzed studies were about preventing pertussis. Regarding the authorship of the studies, epidemiologists participated in 67% of the publications. Conclusion: This study presents the necessary scientific evidence on pertussis during childhood is necessary for prevention, control, promotion, and epidemiological profile of this disease in this population. The results revealed a level of evidence of five, suggesting that the research in this area does not show strong evidence.(AU)


Justificación y Objetivos: La tos ferina todavía es un grave problema de salud a pesar de los esfuerzos realizados por el Programa Nacional de Inmunización. El objetivo de este estudio fue identificar las evidencias científicas en la literatura sobre la tos ferina en la infancia que indican las prácticas de asistencia sanitaria necesarias a la prevención, promoción y control de la enfermedad. Métodos: Revisión integrativa de la literatura en las bases de datos LILACS, MedLine, PubMed, SciELO, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL) utilizando las palabras clave: Whooping Cough, Epidemiology, Vaccination. Se llevó a cabo en mayo de 2020 la búsqueda de artículos publicados en portugués, inglés y español, disponibles en su totalidad y sin delimitación de año. Resultados: Se seleccionaron un total de 1.248 artículos, de los cuales solo se analizaron nueve, en su mayoría norteamericanos; el 55% de los estudios analizados fueron sobre la prevención de la tos ferina. Respecto de la autoría de los estudios, un epidemiólogo participó en el 67% de las publicaciones. Conclusiones: Esta investigación muestra evidencias científicas sobre la tos ferina en la infancia necesarias para la prevención, el control, la promoción de la asistencia y la definición del perfil epidemiológico de la enfermedad en esa población. Los resultados revelaron que el nivel de evidencia se remite a cinco, lo que sugiere que la investigación en el área no muestra evidencias sólidas.(AU)

Abstract Objective: To study the knowledge, attitude, and practice (KAP) regarding pertussis among students in a public university in Malaysia. Material and Methods: This study was a cross-sectional study using convenience sampling to recruit 171 respondents. The data was collected using a self-administered questionnaire comprised of four different parts: the socio-demographic data, the knowledge, the attitude and the practice towards the prevention of pertussis. The data were analyzed using correlation, independent t-test, and ANOVA according to the different study objectives and types of data. Results: More than half of the respondents (67.8%) indicated that they had heard about pertussis. The school or university (59.6%) was the most common source of information, followed by Internet (46.2%). The most of the respondents (43.9%) possessed moderate knowledge regarding pertussis and knew that Bordetella pertussis is the causative agent of pertussis (76.0%). A significant positive correlation between knowledge regarding pertussis with age (p=0.023) was observed, however, there was no association between age with attitude and practice (p=0.272 and 0.131, respectively). Gender and marital status did not influence the KAP regarding pertussis; nevertheless, significantly different between different faculties. Conclusion: Students from the Faculty of Medicine had the highest knowledge scores, while the students from the Faculty of Nursing had the most top attitude and practice scores compared to students from another faculty. The students from the public university generally had a good level of KAP regarding pertussis.

Pertussis, tos convulsa o coqueluche son términos que se emplean como sinónimos para referirse a una infección respiratoria inmunoprevenible grave causada por la bacteria gram negativa denominada Bordetella pertussis. La mejor manera de prevenir la enfermedad es a través de la vacunación. Las primeras experimentaciones con vacunas comenzaron después de que Jules Bordet y Octave Gengou del Instituto Pasteur de Bruselas identificaran el agente etiológico en 1906. Estas primeras vacunas se hicieron a partir de células enteras del agente causal muertas por calor. La historia de las vacunas contra la enfermedad continuó desde aquel entonces con vacunas combinadas y luego con vacunas de componentes o acelulares. Su uso masivo desde los años 50 permitió una reducción muy marcada de la morbimortalidad asociada a la enfermedad. Sin embargo en el año 2008, se estimó que en el mundo se producen por año 16 millones de casos de los cuales 195.000 resultan ser fatales. Para el año 2014 esta estimación sobre el número de casos creció a 24,1 millones de casos en el año. El incremento del número de casos detectado en los últimos 20 años ha estado dirigiendo la mirada de la comunidad sanitaria y científica hacia la identificación de causas de esta nueva situación epidemiológica de pertussis para revisar e implementar estrategias de control más efectivas. Se ha logrado así un mejor reconocimiento de la enfermedad no solo entre los lactantes y los niños, sino también en los adolescentes y adultos. El mayor reconocimiento de que los niños mayores, los adolescentes y los adultos están en riesgo de contraer la enfermedad y que pueden transmitirla a los más vulnerables ha resaltado la necesidad de comprender mejor la inmunidad inducida por las vacunas y su duración. El rol de las vacunas y en particular de las vacunas acelulares constituidas por pocos inmunógenos en altas dosis sobre la selección de geno/fenotipos bacterianos más resistentes a la inmunidad inducida por las vacunas ha comenzado a visualizarse más claramente. La investigación en curso que utiliza herramientas novedosas sin dudas ha mejorado el conocimiento en general sobre esta patología, sin embargo la investigación debe continuar de forma de lograr una vigilancia más oportuna con terapias y vacunas de nueva generación más eficaces.


Pertussis or whooping cough is a preventable respiratory infectious disease caused by the gram-negative microorganism known as Bordetella pertussis. The best strategy to prevent pertussis is to get vaccinated. Vaccine development began just after Jules Bordet and Octave Gengou at Pasteur Institute from Brussels identified the etiologic agent of the disease in 1906. The first vaccine was formulated with heat-killed B. pertussis bacteria, which was later combined with tetanus and diphtheria toxoids (DTP). The second generation of pertussis vaccine was the acellular vaccine consisting in a few purified B. pertussis immunogens. The massive use of these vaccines since the 50s reduced the morbidity and mortality associated with the disease. However, in 2008 it was estimated that 16 million cases occurred by year with 195,000 deaths worldwide. For 2014, this estimation rised to 24.1 million cases per year. The increase in the number of cases detected in the last 20 years has been directing the attention of the health and scientific community towards the identification of causes of this new epidemiological situation of pertussis to review and implement more effective control strategies. This has achieved a better recognition of the disease not only among infants and children but also in adolescents and adults. The awareness that older children, adolescents and adults are at risk of contracting the disease and that they can transmit pertussis to the most vulnerable highlighted the need to better understand the immunity induced by pertussis vaccination and also the duration of such immunity. Another aspect that needs to be understood is that related to the selection pressure that the vaccines would be exerting (in particular the acellular vaccines) on the circulating bacterial population. In this sense, an increase in the prevalence of strains of B. pertussis that are more resistant to the immunity conferred by the vaccines has been detected. The ongoing research using innovative tools has undoubtedly improved the knowledge on pertussis; however research should continue to achieve a more timely surveillance with more effective new generation therapies and vaccines.


Pertussis, tosse convulsa ou coqueluche são termos que se utilizam como sinônimos para fazer referência a uma infecção respiratória imunoprevenível grave provocada pela bactéria gram negativa denominada Bordetella pertussis. A melhor forma de prevenir a doença é através da vacinação. As primeiras experimentações com vacinas começaram depois de que Jules Bordet e Octave Gengou do Instituto Pasteur de Bruxelas identificassem o agente etiológico em 1906. Estas primeiras vacinas foram feitas a partir de células inteiras do agente causal mortas por calor. A história das vacinas contra a doença continuou a partir de então com vacinas combinadas e depois com vacinas de componentes ou acelulares. O uso generalizado delas desde os anos 50 permitiu uma redução muito importante da morbimortalidade associada à doença. Entretanto, no ano 2008, a estimativa foi de 16 milhões de casos produzidos no mundo por ano dos quais 195.000 resultaram fatais. Para o ano 2014, essa estimativa sobre o número de casos cresceu a 24,1 milhões de casos no ano. O aumento do número de casos detectado nos últimos 20 anos dirigiu e dirige o foco da comunidade sanitária e científica para a identificação de causas dessa nova situação epidemiológica de coqueluche de forma de revisar e implementar estratégias de controle mais efetivas. Um melhor reconhecimento da doença foi assim possível, não só entre bebês e meninos, mas também nos adolescentes e adultos. O maior reconhecimento de que as crianças mais velhas, os adolescentes e os adultos estão em risco de contrair a doença e que pode transmiti-la aos mais vulneráveis tem salientado a necessidade de compreender melhor a imunidade induzida pelas vacinas e a duração delas. O papel das vacinas e, em particular, das vacinas acelulares constituídas por poucos imunógenos em altas doses sobre a seleção de genótipos/fenótipos bacterianos mais resistentes à imunidade induzida pelas vacinas tem começado a ser visualizado mais claramente. A pesquisa em andamento que utiliza ferramentas novas, sem dúvidas, tem melhorado o conhecimento em geral sobre essa patologia, contudo a pesquisa deve continuar de maneira de alcançar uma vigilância mais oportuna com terapias e vacinas de nova geração mais eficazes.

ABSTRACT Objective: Through a systematic review, this essay aimed at revising the concepts of severe pertussis, updating the epidemiology, pathophysiology, clinical presentation, antibiotic therapy and auxiliary therapeutic options for symptomatology and complications. Data sources: This review considered publications from the last 30years in the databases US National Library of Medicine (PubMed), Scientific Electronic Library Online (SciELO), Literatura Latino-americana e do Caribe em Ciências da Saúde (LILACS), Cochrane, Google Scholar, as well as protocols of the Ministry of Health and recommendations of the Centers for Disease Control and Prevention, related to childhood pertussis (whooping cough), with emphasis on its severe form. This research was based on keywords derived from the terms "pertussis", "azithromycin", "antitussives", "leukocyte reduction" in Portuguese and English. Duplicate studies and those with unavailable full-text were excluded. Data synthesis: Among 556 records found, 54 were selected for analysis. Pertussis, as a reemerging disease, has affected all age groups, evidencing the transient immunity conferred by infection and vaccination. Severe cases occur in neonates and infants, with secondary viral and bacterial complications and malignant pertussis, a longside hyperleukocytosis, respiratory failure and shock. Macrolides continue to be the chosen antibiotics, while antitussives for coughing remain without efficacy. The prompt treatment in Intensive Care Units improved the prognostic in severe cases, and transfusion was promising among procedures for leukoreduction. Conclusions: Approaching severe pertussis in childhood remains a challenge for diagnostic and therapy, as the available therapeutic options are still unsatisfactory. Strategies of prevention are expected to reduce the occurrence of severe cases, while new studies should confirm the role of auxiliary therapies.


RESUMO Objetivo: Rever os conceitos de coqueluche grave, atualizar epidemiologia, fisiopatologia e apresentação clínica, verificar as recomendações de antibioticoterapia e conhecer opções terapêuticas auxiliares na sintomatologia e complicações, por meio de revisão sistemática. Fontes de dados: Foram pesquisados trabalhos publicados nos últimos 30 anos nas bases US National Library of Medicine (PubMed), Scientific Electronic Library Online (SciELO), Literatura Latino-americana e do Caribe em Ciências da Saúde (LILACS), Cochrane e Google Scholar, bem como protocolos do Ministério da Saúde e recomendações do Centers for Disease Control and Prevention, relacionados à coqueluche na infância, com ênfase na forma grave. Apesquisa baseou-se em palavras-chave derivadas dos termos "coqueluche", "azitromicina", "antitussígenos" e "redução de leucócitos", nos idiomas português e inglês. Foramexcluídos estudos em duplicata ou texto integral indisponíveis. Síntese dos dados: Dos 556 registros encontrados, foram selecionados 54 para análise. A coqueluche, como doença reemergente, tem acometido todas as faixas etárias, evidenciando a imunidade transitória conferida pela infecção e pela vacinação. Quadros graves ocorrem em neonatos e lactentes, com complicações virais e bacterianas secundárias e pertussis maligna, com hiperleucocitose, insuficiência respiratória e choque refratário. Os macrolídeos continuam como antibióticos de escolha. Os sintomáticos da tosse não demonstraram eficácia. O suporte precoce em Unidade de Terapia Intensiva melhorou o prognóstico dos casos graves e a exsanguineotransfusão se mostrou a mais promissora entre os procedimentos para leucorredução. Conclusões: A abordagem da coqueluche grave na infância segue como desafio diagnóstico e terapêutico. As opções terapêuticas disponíveis ainda são insatisfatórias. Espera-se que as estratégias de prevenção reduzam a ocorrência de casos graves e que novos estudos confirmem o papel das terapias adjuvantes.

Abstract Introduction: Serological surveillance (serosurveillance) provides the most direct measure of herd immunity of vaccine-preventable diseases. Little is known about the opportunities and challenges of serosurveillance experiences, particularly pertussis. Objective: To describe the process of serosurveillance for vaccine-preventable diseases with an emphasis on the experience of pertussis in the metropolitan area of Antioquia (Valle de Aburrá) in 2015 and 2016 and analyze the contributions and challenges for its sustainability. Materials and methods: We described the planning and conduction of serosurveillance of pertussis antibodies of mothers and in the umbilical cord at the time of delivery in eight hospitals based on random sampling and their capacity to advance the serosurveillance periodically. We compared the contributions and the challenges of this experience with other probabilistic and non-probabilistic programs. Results: We achieved the participation of hospitals and mothers respecting the delivery care process. We established a serum bank following ethical and technical guidelines. This program based on the random selection of hospitals and mothers has enabled the estimation of antibodies prevalence in mothers and in the umbilical cord, which has been possible given the high coverage of hospital care during childbirth at a lower cost and fewer risks than a population-based survey in conflictive areas. The main challenges for the sustainability of this program are the creation of stable jobs and access to funding and legal and methodological long-term frameworks. Conclusions: Hospital serosurveillance as described is an option to monitor the impact of vaccination on the population. Our experience could be reproduced in other regions under similar conditions if the above-mentioned challenges are solved.


Resumen Introducción. La vigilancia serológica es la forma más directa de medir la inmunidad de rebaño frente a las enfermedades prevenibles por vacunación. Poco se sabe acerca de las oportunidades y los desafíos de las experiencias de serovigilancia, en general y, específicamente, la de la tosferina. Objetivo. Describir el proceso de serovigilancia de enfermedades prevenibles por vacunación con énfasis en la experiencia en el caso de la tosferina en el área metropolitana de Antioquia (Valle de Aburrá) en el 2015 y el 2016 y analizar lo que dicha experiencia ha aportado y los desafíos que persisten para su sostenibilidad. Materiales y métodos. Se describió el proceso de planeación y el desarrollo de la serovigilancia de tosferina en el momento del parto en ocho hospitales seleccionados al azar, así como la capacidad para adelantar el programa de manera periódica. Se compararon los aportes y los desafíos en el curso de esta experiencia con los de otros programas poblacionales probabilistas e institucionales no probabilistas. Resultados. Se logró la participación de los hospitales y de las madres con pleno respeto del proceso de atención del parto, y se conformó un banco de sueros siguiendo lineamientos éticos y técnicos. El programa permitió estimar la prevalencia de anticuerpos en la madre y en el cordón umbilical, lo que se facilitó por la alta cobertura de atención hospitalaria del parto, a un menor costo y menos riesgos que los programas poblacionales en zonas conflictivas. Los principales desafíos para la sostenibilidad del programa son la estabilidad laboral del personal de salud, así como normas y una financiación de largo plazo. Conclusiones. La serovigilancia hospitalaria es una opción para monitorizar el impacto poblacional de la vacunación. Esta experiencia se podría extender a otras regiones en condiciones similares si se resuelven los retos mencionados.

RESUMEN Objetivo: Describir las características clínicas y epidemiológicas de los pacientes menores de 2 años hospitalizados con el diagnóstico de tos ferina en un hospital pediátrico de tercer nivel de Perú. Métodos: Serie de casos de pacientes menores de 2 años hospitalizados con diagnóstico de tos ferina durante el año 2012. Resultados: Fueron hospitalizados 121 pacientes. Se realizaron pruebas para confirmar el diagnóstico (inmunofluorescencia directa, reacción en cadena de la polimerasa, cultivo) al 53,72%. El 23,15% (n = 28) fueron casos confirmados, todos menores de 10 meses, ninguno había recibido 3 dosis de la vacuna contra pertussis, el 96,43% (n = 27) de ellos fueron menores de 6 meses y 42,86% (n = 12) menores de 3 meses; un 10,71% (n = 3) ingresaron a unidad de cuidados intensivos, todos menores de 2 meses, uno de los cuales falleció. Los síntomas más frecuentes en los casos confirmados fueron tos (96,43%), rubicundez facial (96,43%), tos paroxística (92,86%) y cianosis asociada a la tos (78,57%); el contacto epidemiológico probable más frecuente fue la madre (17,86%) y la mayoría de casos se presentaron en verano (46,43%). Conclusión: La tos ferina es causa de morbimortalidad sobre todo en los menores de 6 meses de edad y en los no inmunizados o parcialmente inmunizados. Se deben mejorar las tasas de vacunación y fomentar la confirmación de casos para no contribuir al infradiagnóstico de esta enfermedad.


ABSTRACT Objective: Describe the clinical and epidemiological characteristics of patients under 2 years of age hospitalized with whooping cough in a tertiary care children's hospital in Peru. Methods: This was a case series of patients under 2 years of age who were hospitalized with a diagnosis of whooping cough in 2012. Results: A total of 121 patients were hospitalized. Diagnostic testing (direct immunofluorescence, polymerase chain reaction, culture) was carried out in 53.72% of patients. Overall, 23.15% (n = 28) were confirmed cases, all of whom were patients less than 10 months old, and none of whom had received 3 doses of whooping cough vaccine. A total of 96.43% (n = 27) of cases were under 6 months of age, 42.86% (n = 12) were younger than 3 months, and 10.71% (n = 3) were admitted to the intensive care unit. Of these cases, all were younger than 2 months old, and one patient died. The most common symptoms in the confirmed cases were coughing (96.43%), facial redness (96.43%), paroxysmal coughing (92.86%), and coughing-related cyanosis (78.57%). The most frequent probable epidemiological contact was the mother (17.86%), and the majority of cases occurred in the summer (46.43%). Conclusion: Whooping cough is a cause of morbidity and mortality, especially in those younger than 6 months of age and in those who are not immunized or only partially immunized. Vaccination rates should be improved and case confirmation encouraged to prevent the underdiagnosis of this disease.

Resumen Introducción Bordetella pertussis es el agente causal de la tosferina, una enfermedad de alta letalidad, especialmente en menores de 6 meses, pero prevenible mediante la vacunación. Los reportes en hospitales de brotes de tosferina muestran que el caso índice suelen ser personas adultas. En adultos, la enfermedad se manifiesta principalmente con tos persistente. El propósito de este estudio fue conocer la seroprevalencia de B. pertussis en el personal de salud de un hospital pediátrico en un país donde aún no se considera la vacunación obligatoria para los empleados. Métodos Participaron personal de enfermería y médicos residentes en trato directo con pacientes hospitalizados. A cada participante se le realizó detección de anticuerpos inmunoglobulina G, antitoxina de pertussis (anti-TP) y se le aplicó un cuestionario para datos clínicos y demográficos. Resultados Se incluyeron 93 individuos, el 85% de personal de enfermería con mediana de edad de 35 años (rango intercuartil: 29-42.5). El 21.5% de los participantes laboraban en el Servicio de Urgencias, el 8.6%, en la Unidad de Terapia Intensiva Pediátrica, el 6.5%, en la Unidad de Cuidados Intensivos Neonatales. Se encontraron títulos detectables de anticuerpos anti-TP en el 18.3%, de los cuales, el 53% presentaron títulos de infección reciente y solamente el 23.5%, historia de tos de más de dos semanas de evolución. Conclusiones El personal de salud está en riesgo de sufrir la enfermedad y de transmitirla a los lactantes, quienes pueden fallecer por esta causa. Este estudio sugiere que las políticas actuales de vacunación en personal de salud se deben de modificar para determinar obligatoriedad de la vacuna, especialmente en quienes atienden a la población pediátrica.


Abstract Background Bordetella pertussis is the causative agent of pertussis, a disease that is preventable by vaccination but has a high mortality, particularly in children < 6 months. Reports of pertussis outbreaks in hospitals show that the index case is usually an adult. In adults, the disease manifests mainly with persistent cough. The purpose of this study was to determine the seroprevalence of B. pertussis in the health personnel of a pediatric hospital in a country where vaccination of this staff is not considered mandatory. Methods Nursing staff and resident doctors who were involved in direct treatment with hospitalized patients participated in the study. Each participant was screened for immunoglobulin G anti-pertussis toxin antibodies (anti-PT), and a questionnaire was applied for clinical and demographic data. Results Ninety-three individuals were included, of which 85% were nurses, median age 35 years (interquartile range: 29-42.5). The participants worked in the emergency department (21.5%), in the Pediatric Intensive Care Unit (8.6%), and in the Neonatal Intensive Care Unit (6.5%). Detectable titers of anti-TP antibodies were found in 18.3%, of which 53% presented titles suggestive of recent infection and only 23.5% cough > 2 weeks of duration. Conclusions Health personnel are at risk of suffering from the disease and be potential transmitters to infants, who may die from this cause. This study suggests that the current vaccination policies in health personnel should be modified to determine the compulsory nature of the vaccination, especially in those individuals in charge of the care of the pediatric population.

ABSTRACT

O objetivo deste estudo foi de confirmar laboratorialmente os casos suspeitos de coqueluche na região oeste do Estado de São Paulo, ocorridos entre 2010 a 2015. A cultura foi realizada no Centro de Laboratório Regional - Instituto Adolfo Lutz de Presidente Prudente e a PCR em tempo real (qPCR) foi realizada no Centro de Referência Nacional para Pertussis – Instituto Adolfo Lutz em São Paulo, SP. Foram recebidas 189 amostras, sendo 29 (15,3%) confirmadas segundo os critérios laboratoriais (cultura e/ou qPCR). A faixa etária mais acometida foi em crianças menores de seis meses de idade (82,8%), não vacinados ou com o esquema de vacinação incompleto. Provavelmente, estes resultados representam apenas uma fração do número real de casos de coqueluche que ocorrem no Brasil. O contínuo monitoramento da doença e informações da prevalência por faixa etária são importantes ferramentas para melhorar as estratégias de imunização como forma de controlar esta doença reemergente.


The aim of this study was to confirm the suspected cases of pertussis in the Western region of the Sao Paulo State from 2010 to 2015. The samples were cultured in the Instituto Adolfo Lutz - Regional Laboratory of Presidente Prudente-SP, and the qPCR was performed at the National Reference Laboratory for Pertussis – Central Instituto Adolfo Lutz, São Paulo-SP. In this period, 189 samples were received, being 29 (15.3%) confirmed by the laboratory criteria (culture and/or qPCR). The most affected group was the children less than six months old (82.8%), not vaccinated or with the incomplete vaccination. Most likely, these results only represent a fraction of the actual number of pertussis cases occurring in Brazil. The continuous disease monitoring and the prevalence data by age group are fundamental to improve the immunization strategies as a way to control this important re-emerging disease.

Introducción. La tos convulsa es una enfermedad altamente contagiosa causada por Bordetella pertussis. Tiene una alta tasa de morbilidad y mortalidad, especialmente, en los lactantes menores de seis meses de edad. En la Argentina, la incidencia y la mortalidad se han encontrado en aumento en las últimas 3 décadas. Objetivo. Determinar anticuerpos contra Bordetella pertussis en las mujeres embarazadas en el tercer trimestre de la gestación y en el recién nacido, medidos en la sangre del cordón. Métodos. Se disenó un estudio observacional, transversal. El estudio se inició en 2011 cuando la vacunación contra pertussis en la embarazada no estaba incluida en el Calendario Nacional de Vacunación y era opcional. Los anticuerpos se midieron en las madres en el tercer trimestre del embarazo y en la sangre del cordón umbilical al nacer. Las determinaciones de anticuerpos se realizaron con el kit de ELISA humano para IgG toxina pertussis ABCAMR. Se utilizó la prueba de chi² para comparar la prevalencia. Resultados. Se incluyó a 111 madres y a sus bebés, 35 hijos de madres no vacunadas (antes de la implementación de la vacuna en embarazadas) y 76 hijos de madres vacunadas. Los bebés de madres vacunadas presentaron anticuerpos IgG positivos en el 92% (70/76), mientras que los bebés de madres no vacunadas fueron negativos para anticuerpos IgG en el 100% (35/35) con una p < 0,001. Conclusión. En la población de vacunadas del estudio, se observó que sus hijos presentaron anticuerpos IgG positivos en el 92%. Este estudio apoya la necesidad de la inmunización materna contra Bordetella pertussis para proteger al recién nacido.


Introduction. Pertussis is a highly contagious disease caused by Bordetella pertussis. It poses a high morbidity and mortality rate, especially among infants younger than 6 months old. In Argentina, pertussis incidence and mortality have increased over the past three decades. Objective. To establish Bordetella pertussis antibody titers among pregnant women in their third trimester and among newborn infants, as measured in cord blood. Methods. This was an observational, crosssectional study. The study started in 2011; at that time, pertussis vaccination was not mandatory for pregnant women as per the national immunization schedule, only optional. Maternal antibodies were measured in the last trimester of pregnancy for women and in cord blood for newborn infants. Antibody titers were determined using Abcam's anti-Bordetella pertussis toxin (PT) IgG in vitro ELISA kit. The X2 test was used to compare prevalence rates. Results. The study included 111 mother-newborn infant dyads; 35 infants from unvaccinated mothers (before the introduction of the vaccine) and 76 from vaccinated mothers. Positive IgG antibodies were found in 92% (70/76) of infants born from vaccinated mothers whereas 100% (35/35) of infants born from unvaccinated mothers had negative results for antibodies; p < 0.001. Conclusion. In the vaccinated population of this study, 92% of infants had positive IgG antibodies. This study supports the need for maternal immunization against Bordetella pertussis to provide protection to newborn infants.

El coqueluche es una enfermedad inmunoprevenible que puede amenazar la vida. A pesar de la vacunación en lactantes desde los 2 meses de edad, se siguen presentando casos y brotes a nivel nacional y mundial, con un alto riesgo de morbimortalidad en los menores de 6 meses. Se ha planteado que la vacunación antenatal con componente pertussis acelular (Tdpa) sería útil, segura y efectiva, dado que transfiere una alta tasa de anticuerpos al niño, disminuyendo en 85% la incidencia de coqueluche en este grupo. No se ha encontrado mayor incidencia de efectos adversos en embarazadas con esta vacuna. Esta estrategia ha sido implementada en varios países desarrollados y de Latinoamérica. El objetivo del presente manuscrito es revisar y discutir los beneficios de la vacunación antenatal con Tdpa. Se concluye que se debiera promover la inmunización materna con vacuna Tdpa para prevenir la infección y mortalidad asociada en el menor de 6 meses de vida por Bordetella pertussis.


Whooping cough is an immune preventable disease that can be life threatening. Despite infant immunization starting at 2 month of age, there are many cases and outbreaks in our country and also around the world, with a high risk of mortality specially in infants under 6 month of age. It has been proposed that antenatal vaccination with acellular pertussis component (Tdap) would be useful, safe and effective since it transfers a high antibody rate to the child, reducing the incidence of pertussis in this group by 85%. No higher incidence of adverse effects has been found in pregnant women with this vaccine. This strategy has been implemented in several developed and Latin American countries. The purpose of this manuscript is to review and discuss the benefits of antenatal vaccination with Tdap. It was concluded that maternal immunization with Tdap vaccine should be promoted to prevent infection and associated mortality in the less than 6 months of age by Bordetella pertussis.

Introduction: Whooping cough is a re-emerging infection in the world and Latin America. Objective: It was considered relevant to investigate the clinical and epidemiological profile of Bordetella spp. and Bordetella pertussis infection in Córdoba province, Argentina; evaluating, at the same time, the co-infection with virus producing respiratory infections that may be confused with whooping cough. Material and Methods: All whooping cough suspected cases were studied by Polimerase Chain Reaction, amplifying the repeated insertion sequence (IS) 481 and the promoter gene encoding pertussis toxin, between 2011 and 2013. The data were obtained from the clinical and epidemiological records. Results: From 2,588 whooping cough suspected cases, 11.59% was infected by Bordetella spp. and 9.16% was confirmed as Bordetella pertussis infection. The rate of infection was 7.22 and 1.84 per 100,000 for 2011 and 2012, respectively. The infection presented a seasonal tendency and it was mainly found on the group of children between 13 and 24 months old. The co-infection with virus producing respiratory infections, were uncommon. Paroxysmal cough, cyanosis and/or vomiting were predictors of the infection for Bordetella pertussis. Discussion and Conclusions: To deal with the re-emergence of whooping cough is important the knowledge of the regional epidemiological situation. This paper shows the situation of these infections in the regional clinical and epidemiological context, and makes the information available for health decision-making.


Introducción: Coqueluche es una enfermedad reemergente en el mundo y en Latinoamérica. Objetivo: Resultó de interés caracterizar el perfil clínico-epidemiológico de la infección por Bordetella spp. y Bordetella pertussis en Córdoba, Argentina; evaluando además, la frecuencia de infecciones de etiología viral que, por cursar con un síndrome coqueluchoide (SC), pueden ser confundidas con cuadros de coqueluche. Material y Métodos: Los casos sospechosos de coqueluche, se estudiaron por reacción de polimerasa en cadena; amplificando la secuencia repetida de inserción (IS) 481 y la región promotora del gen de la toxina pertussis; entre 2011 y 2013. Los datos de los pacientes se obtuvieron de las fichas clínicoepidemiológicas. Resultados: De 2.588 pacientes, 11,59% presentó una infección por Bordetella spp. y en 9,16% se confirmó una infección por Bordetella pertussis. La tasa de infección fue 7,22 y 1,84 por 100.000 habitantes en 2011 y 2012, respectivamente. La infección presentó una tendencia estacional y se concentró principalmente en niños entre 13 y 24 meses. La tos paroxística, cianosis y/o vómitos fueron predictores de la infección por B. pertussis. La coinfección con virus productores de infecciones respiratorias fue poco frecuente. Discusión y Conclusiones: Es fundamental el conocimiento de la situación epidemiológica regional. Este trabajo presenta la situación de Córdoba y pone a disposición de la comunidad sanitaria la información para la toma de decisiones en el contexto clínico-epidemiológico regional.

RESUMEN Objetivos Caracterizar la población en estudio en el marco del Sistema Nacional de Vigilancia de Salud de la Argentina, determinar la proporción de infección por Bordetella pertussis y Bordetella parapertussis e identificar factores asociados de los casos con sospecha de coqueluche atendidos en la ciudad de Mar del Plata y alrededores durante el período 2011- 2015. Materiales y Métodos Se realizó un estudio observacional y descriptivo. Se diagnosticaron por laboratorio casos clínicos con sospecha de coqueluche. Los estudios de laboratorio consistieron en cultivo, PCR y serología por técnica de ELISA. Resultados Se evaluaron 572 casos. El sexo femenino fue el más frecuente con 51,9% (296/570), el rango etario más frecuente fue de 2 a 17 meses con 51,1% (290/568) que fue además el grupo con más casos confirmados. Solo el 47,8% (155/324) de la población estudiada tenía vacunación completa para su edad. Se confirmó coqueluche por Bordetella pertussis en el 15,5 % y un solo caso con Bordetella parapertussis. Hubo asociación entre tener un contacto familiar tosedor por más de dos semanas con la confirmación de Bordetella spp. (odds ratio [OR]: 3,3; [intervalo de confianza al 95%] IC 95%: 1,9 - 5,4). Conclusiones Existe la necesidad de realizar una evaluación oportuna de los casos sospechosos de coqueluche tanto en niños como en adolescentes y adultos para lograr un mejor control de la enfermedad. Asimismo, continuar con las medidas de prevención y de contención resulta fundamental para disminuir la circulación del agente causal.


ABSTRACT Objectives . To classify the study population in the Argentinian National Health Surveillance System framework, determine the proportion of infection by Bordetella pertussis and Bordetella parapertussis, and identify factors associated with the cases of suspected whooping cough attended to in the city of Mar del Plata and its outskirts during the period 2011- 2015. Materials and Methods An observational and descriptive study was carried out. Clinical cases with suspicion of whooping cough were diagnosed by laboratory. The laboratory studies consisted of culture, PCR, and serology using the ELISA technique. Results A total of 572 cases were evaluated. The female sex was the most frequent (51.9%). The most frequent age range was 2 to 17 moths (51.1%; 290/568), which was also the group with the most confirmed cases. Only 47.8% (155/324) of the population studied had complete vaccination for their age. Whooping cough due to B. pertussis was confirmed in 15.5% (89/572) of cases and one case with B. parapertussis. Those cases that had contact with a coughing relative were significantly associated with the confirmation of Bordetella spp. by the laboratory (odds ratio: 3.3; 95% confidence interval: 1.9-5.4). Conclusions The results show the need to suspect whooping cough and diagnose it early in children, adolescents, and adults in order to better control the disease. Likewise, continuing prevention and containment measures are fundamental in decreasing the circulation of the causal agent.

ABSTRACT

Objectives In Latin America and the Caribbean (LAC), pertussis disease incidence has reportedly increased since 2000 despite high vaccine coverage. A systematic review of pertussis literature and a meta-analysis was conducted to understand the burden of disease in LAC. Methods A systematic literature review was completed, using relevant search terms. Original articles describing pertussis epidemiology and vaccine coverage in LAC published between 1980 and 2015 were identified. Applying a Bayesian meta-analysis random-effects model, we calculated pooled estimates and corresponding 95% credible intervals (95% CrIs) for pertussis incidence, case fatality ratio (CFR), pertussis prevalence among contacts, and coverage with three doses of diphtheria, tetanus, and pertussis (DTP) vaccine (DTP3). Results A total of 59 studies meeting our selection criteria were identified, representing 15 countries. Of the 59, 15 of them provided incidence data, with 7 of the 15 reporting a pertussis case definition. The pertussis incidence estimate for the 1980-1999 period was 17.8 cases per 100 000 persons (95% CrI: 5.9-29.7); for the 2000-2015 period, it was 2.5 cases per 100 000 persons (95% CrI: 1.8-3.2). For the 1980-2015 period, the CFR, in 19 studies reviewed, was 3.9% (95% CrI: 2.9%-4.9%); for that same period, in 5 studies reviewed, pertussis prevalence among contacts was 24.9% (95% CrI: 13.7%-36.1%). Pooled DTP3 vaccine coverage estimates, in a total of 20 studies reviewed for the following three time periods, were: 1980-1990, 72.4% (95% CrI: 64.6%-80.2%); 1991-2000, 79.0% (95% CrI: 66.1%-91.9%); and 2001-2015, 90.0% (95% CrI: 87.7%-92.3%). Conclusion A decrease in pertussis incidence and an achievement of moderately high DTP3 vaccine coverage since the early 2000s was observed. The review highlights the need for increased publication of pertussis data at the country level and for LAC as a whole in order to better understand the true burden of the disease. Application of a standardized case definition and use of active case finding would aid in obtaining more accurate estimates of the disease burden in LAC.


RESUMEN Objetivos En América Latina y el Caribe, la incidencia de la tos ferina habría aparentemente aumentado desde el 2000, a pesar de la alta cobertura de vacunación. Se realizó una revisión sistemática de la bibliografía sobre tos ferina y un metanálisis para conocer la carga de esta enfermedad en América Latina y el Caribe. Métodos. La revisión bibliográfica sistemática se realizó utilizando términos de búsqueda pertinentes. Se encontraron artículos originales que describían las características epidemiológicas de la tos ferina y la cobertura de vacunación en América Latina y el Caribe publicados entre 1980 y el 2015. Aplicando un modelo bayesiano de efectos aleatorios para el metanálisis, se obtuvieron estimaciones combinadas y los correspondientes intervalos de credibilidad del 95% (ICr) para la incidencia, la tasa de letalidad y la prevalencia entre contactos de la tos ferina, y la cobertura con tres dosis de vacuna contra la difteria, el tétanos y la tos ferina (DTP3). Resultados Se encontraron en total 59 estudios de 15 países que cumplían con los criterios de selección. De ellos, 15 proporcionaban datos sobre incidencia. Siete de estos 15 contenían una definición de caso de tos ferina. La incidencia estimada de tos ferina para el período 1980-1999 fue de 17,8 casos por 100 000 (ICr 95%: 5,9-29,7) y para el período 2000-2015, de 2,5 casos por 100 000 (ICr 95%: 1,8-3,2). En cuanto a la tasa de letalidad, para el período 1980-2015 en 19 estudios examinados fue de 3,9% (ICr 95%: 2,9%-4,9%); en el mismo período, en 5 estudios examinados la prevalencia de tos ferina entre los contactos fue de 24,9% (ICr 95%: 13,7%-36,1%). Las estimaciones combinadas de cobertura con DTP3 en un total de 21 estudios examinados para los siguientes tres períodos fueron: en 1980-1990, 72,4% (ICr 95%: 64,6%-80,2%); en 1991-2000, 79,0% (ICr 95%: 66,1%-91,9%) y en el 2001-2015, 90,0% (ICr 95%: 87,7%-92,3%). Conclusiones Se observó una disminución de la incidencia de la tos ferina y el logro de una cobertura moderadamente alta con la vacuna DTP3 desde principios del siglo XXI. En el examen se subraya la necesidad de incrementar la publicación de datos sobre la tos ferina en los países y en América Latina y el Caribe en su conjunto, para conocer mejor la verdadera carga de enfermedad. La adopción de una definición de caso normalizada y la búsqueda activa de casos ayudarán a obtener estimaciones más precisas de la carga de enfermedad en América Latina y el Caribe.


RESUMO Objetivos Há relatos de que a incidência de coqueluche na América Latina e Caribe (ALC) tem aumentado desde o ano 2000, apesar da alta cobertura vacinal. Realizamos uma revisão sistemática e metanálise da literatura sobre a coqueluche para compreender a carga da doença na ALC. Métodos. Fizemos uma revisão sistemática da literatura usando termos de pesquisa relevantes. Identificamos artigos originais, publicados entre 1980 e 2015, que descrevessem a epidemiologia da coqueluche e a cobertura vacinal na ALC. Aplicando um modelo Bayesiano de efeitos aleatórios para a metanálise, calculamos estimativas agrupadas e seus correspondentes intervalos de credibilidade de 95% (CrIs 95%) para a incidência de coqueluche, a taxa de letalidade, a prevalência de coqueluche entre os contatos e a cobertura com três doses da vacina combinada para difteria, tétano e coqueluche (DTP3). Resultados Identificamos um total de 59 estudos que cumpriram os nossos critérios de seleção, representando 15 países. Destes 59, 15 apresentaram dados sobre a incidência, e 7 dos 15 apresentaram uma definição de "caso de coqueluche". A incidência estimada da coqueluche no período de 1980 a 1999 foi de 17,8 casos por 100.000 pessoas (CrI 95%: 5,9-29,7); no período de 2000 a 2015, foi de 2,5 casos por 100.000 pessoas (CrI 95%: 1,8-3,2). No período de 1980 a 2015, a taxa de letalidade, em 19 estudos revistos, foi de 3,9% (CrI 95%: 2,9%-4,9%); neste mesmo período, em 5 estudos revistos, a prevalência de coqueluche entre os contatos foi de 24,9% (CrI 95%: 13,7%-36,1%). A cobertura vacinal agrupada com DTP3, em um total de 21 estudos examinados nos seguintes três períodos, foi estimada em: 1980 a 1990, 72,4% (CrI 95%: 64,6%-80,2%); 1991 a 2000, 79,0% (CrI 95%: 66,1%-91,9%); 2001 a 2015, 90,0% (CrI 95%: 87,7%-92,3%). Conclusões Foi observada uma redução na incidência de coqueluche e uma cobertura vacinal com DTP3 relativamente alta desde o início da década de 2000. Esta revisão destaca a necessidade de melhorar a publicação de dados sobre a coqueluche ao nível nacional e na ALC como um todo, a fim de promover uma melhor compreensão sobre a verdadeira carga da doença. O uso de uma definição padronizada de "caso de coqueluche" e a busca ativa de casos ajudaria na obtenção de estimativas mais precisas da carga da doença na ALC.

ABSTRACT

In recent years there have been Pertussis outbreaks not seen in the last 50 years affecting adults, adolescents and children and causing deaths in young unvaccinated infants. In Chile an outbreak of Pertussis started in year 2011, leaving 16 infants less than 3 months dead during this year, twice the number seen in a non epidemic year. These children were infected before receiving the programmatic vaccines indicated at 2, 4 and 6 months of age, usually from close contacts, especially their mothers. Pertussis control has not been possible for several reasons, such as limited immunity duration of available vaccines and their poor impact on nasopharyngeal carriage, situation that keeps the agent's circulation and transmission, condition often asymptomatic or unrecognized. Additionally, the use of acellular vaccines appears to be a determining factor because they induce an immune response with poor immune memory and consequently a short time duration. The acellular vaccines with reduced antigen content, available for adolescents and adults, has allowed the evaluation of various strategies but none has succeeded in reducing infant mortality. Recently a new strategy of vaccinating pregnant women against Pertussis in the second or third semester has shown remarkable results reducing up to 90% infant deaths due to Pertussis infection. This strategy prevents mother's infection avoiding child infection through respiratory droplets and also provides the child with antibodies from placental transmission. Improved pertussis vaccines are required, in the meantime the Committee considers that the pregnant immunization strategy, between the 27 and 36 weeks, with acellular pertussis vaccine should be included in our national vaccine program.


En los últimos años se han observado brotes de coqueluche no vistos en 50 años, afectando a adultos, adolescentes y escolares y provocando muertes en lactantes pequeños aún no vacunados. En Chile, en 2011 se inicia un brote con 16 lactantes bajo 3 meses de edad, fallecidos ese año el doble de muertes de un año no epidémico. Estos niños se infectan antes de recibir las vacunas programáticas de los 2, 4 y 6 meses de edad, a partir de contactos cercanos, en especial de su propia madre. El control de la coqueluche no ha sido posible por varias razones, como la duración acotada de la inmunidad de las vacunas disponibles y su escaso impacto en la portación nasofaríngea que mantiene la circulación y trasmisión del agente, con frecuencia en forma asintomática o no reconocida. Adicionalmente, el uso de las vacunas acelulares pareciera ser un factor determinante pues inducen una respuesta inmune con una pobre memoria inmunológica y por ello de duración más breve. La disponibilidad de vacunas acelulares de contenido antigénico reducido para adolescentes y adultos ha permitido ensayar diversas estrategias, pero ninguna ha logrado reducir la mortalidad de los lactantes. Más recientemente se ha ensayado la vacunación de la madre en el 2° o 3er trimestre del embarazo con resultados notables pues ha disminuido en más de 90% las muertes de lactantes por coqueluche. Esta medida evita la infección de la madre y con ello su transmisión por vía respiratoria al niño y permite transferir anticuerpos por vía placentaria. En espera de mejores vacunas, este comité considera que la inmunización de la mujer embarazada, idealmente entre las 27 y 36 semanas, con vacuna anti-coqueluche acelular debiera ser incluida en nuestro país en forma programática.
